Camp Forms


Camp forms specific to each camp are made available on the respective “camp forms” tab for each camp on the rowing camps page. The “camp forms” tabs will appear on specific camps’ pages at least six weeks prior to camp start.

Our waiver and residency agreement/code of conduct are emailed to you at time of registration by RegattaCentral via DocuSign. These should be filled out online and cannot be accepted as printed forms. Please email us if you do not receive them after registering.

The camps form tabs hold the information packet for each camp, health history, travel, and goals forms alongside any additional forms specific to each camp.

Information packets are for your reference, but health histories must be printed and brought to camp. Travel and goals forms are to be completed on the camps form tab so we can prepare for campers’ arrival at camp. Campers traveling long distances and seeking accommodations the night before camp should contact us via email. All other forms for camps in the U.S. should be brought to camp and handed in at registration; international campers may return other forms via e-mail to international@sparksconsult.com

If forms are not returned by camp registration, the athlete will be unable to participate.

Forms returned via “snail mail” will not be accepted.
